Thank you for choosing Bark City for your pet's grooming services. We strive to maintain a safe and pleasant grooming experience for your pet while in our care. Please take a moment to read and sign our agreement below

"Fit & Healthy"(Required)

You acknowledge your pet is healthy and cleared by your veterinarian for grooming services.

Vaccinations(Required)

Pets, over the age of 4 months, in our care for grooming services are only required to be up to date on the rabies vaccine as per NH state law. All other vaccines are highly recommended for your pet’s health and safety. You agree and understand you will not hold Bark City liable if your pet contracts vaccine-preventable diseases while in our care.

Matted Pet(Required)

Bark City will do its best to remove tangles, mats, webbing and pelted coats as painlessly as possible. During the removal process pre-existing conditions such as open or old wounds, abrasions, irritated skin, hair loss, parasites, etc, may be revealed or noticeable when otherwise covered up by hair. As well as post removal, the formation of hematomas as the blood returns to the constricted areas such as on the ears. You will not hold Bark City liable for irritations and/or medical expenses caused by matting present before the groom and for the medical conditions that arise from removal of said mats.

Parasites(Required)

If your pet, at any time, is found to have fleas on skin or coat there will be an additional grooming treatment and charge of $15. You will not hold Bark City liable for contracting parasites while in our care, nor irritations caused by the parasites.

Geriatric Pet(Required)

Aged and elderly pets require extra care to ensure their experience is safe and painless. Bark City deems dogs over the age of 7 to be “senior” and at a higher risk for underlying illnesses to become aggravated or worse during the grooming process. You agree not to hold Bark City liable for pre-existing conditions.

Special Conditions(Required)

You acknowledge your pet has special conditions which we try everything we can to accommodate. These are not limited too: Arthritis, Diabetes, Cancer, Epilepsy, injuries present before the groom, hip dysplasia, enlarged heart, etc. You will not hold Bark City liable for the worsening of these conditions.

Medical Authorization(Required)

You authorize Bark City to seek emergency medical attention if required, whether due to injury or medical condition while your pet is in our care.

I affirm that I am the rightful legal owner of the pet for which services are being rendered. I authorize this contract to be valid approval for future grooming services, permitting Bark City LLC to accept telephone reservations for service without additional signed contracts or written authorization. I understand that pricing is subject to change. I understand that signing below confirms that I have read, acknowledged and agree to the above Terms of Service.
